Expanding New Braunfels’ primary care network: Inside Pak Medical Group’s renewed vision

Led by Dr. Ho Pak, a geriatrician with more than three decades of experience, the practice was built to offer long-term relationships, continuity and trust in a health care system that can often feel fragmented. However, over the past several years, changes in the business structures supporting the practice influenced patient care priorities, leaving many community members confused about the transitions. Fall 2022-January 2023: lnnovaCare Health increased in size and added additional investment to focus on expanding their footprint and developing a market- leading strategy related to care management. 2023-2024: Pak Medical Group and lnnovaCare began to develop the Texas locations and practices to seek a structure better aligned with its patient-first approach. Early 2025: As changes to the Management Services Organization and health care markets continued, lnnovaCare Health transitioned their focus, restoring the previous leadership. Dr. Pak returned with a strict mandate to rebuild and refocus the market. “I want to finish what I started, which is to build out a great primary care network for the community,” Dr. Pak said. Pak Medical Group by lnnovaCare Health is now operating with a shared vision; one centered on service and a commitment to the New Braunfels community. What does the new practice model focus on? Dr. Pak often explains the practice’s approach through a framework shaped by decades

of patient experience: access,

communication, coordination and technology. Access means patients are able to receive great primary care. Communication extends beyond the visit itself, before and after appointments. Coordination ensures patients aren’t left navigating referrals, prescriptions or insurance approvals alone. Technology can make the patient experience smoother. How is Al being integrated without losing the human side of health care? Artificial intelligence plays a growing role in streamlining non-clinical tasks, such as appointment scheduling and medication requests. For patients, Al-powered tools and wearables can offer new ways to better understand their own health, from monitoring glucose levels to tracking real-time data that informs daily choices. “If I give [patients] the tools to really monitor themselves and to hold themselves accountable, I think outcomes are going to be much better,” Dr. Pak said. However, that doesn’t mean the human side of health care is fading. Dr. Pak emphasized that these efficiencies help create more capacity for nurses and physicians to address clinical concerns directly, providing an even better patient experience. A look at Dr. Pak’s podcast Dr. Pak recently launched a podcast called Beyond the Exam Room with Dr. Pak, which can be

found on YouTube, Instagram and TikTok. The podcast is built from lessons gathered during more than 25 years of patient conversations. The show explores topics such as relationships, becoming a grandparent, grief and more, drawing from real experiences shared in moments of trust. “I started creating a memoir of patient stories,” Dr. Pak said. “I have two dozen patients in my life that have celebrated their diamond anniversary, which is 60 years of marriage. I’ve taken these golden morsels of valuable information ... [from] actual, real-life experiences, and ... I want to share them through the podcast.” Looking ahead, Dr. Pak’s focus is on building greater consistency and excellence across the practice. After earning clinic-of- the-year honors multiple times in the past, he said the goal is to grow and continue to operate to that standard. With leadership aligned and systems refocused on a shared goal of excellence and commitment, Pak Medical Group by lnnovaCare Health is entering the next chapter with clarity, stability and patients squarely at the center, with the continued commitment of “People First, Excellence Always.”
